If you’ve changed your name and you’ve taken an examination through the National Board of Surgical Technology and Surgical Assisting (NBSTSA), follow the steps below to update your name in our records:
What you need to do:
- Login to your account: Use the Login button in the upper-right menu, then go to Maintenance Applications and click Begin to start your name-change request. You can securely upload your supporting documents directly through the portal.
- Submit one of the following legal proof documents showing your new name:
- Marriage certificate
- Divorce decree (only the page showing the name change and official seal)
- Driver’s license
- Passport
- Court order or naturalization documentation
There is no fee for updating your name in our database.
Additional Information:
- If you currently have an open Authorization to Test (ATT) period, please let us know so we can notify the testing company of your name change. If your name on the documents and on file do not match, you WILL be prevented from entering the examination.
- If you’re also submitting an Examination or Renewal Application at the same time, you may skip the separate Name Change form and simply include one of the required legal documents with your application.
